Patients who suffer from a serious illness and who require medical treatment abroad can benefit from the help provided by the Romanian National Health Insurance House, through S2 Form (formerly E 112). It is issued on the basis of European Council Regulation (EEC) no. 1408/71 on the application of social security schemes to employed persons, to self-employed persons and to their families moving within the Community, and to European Council Regulation (EEC) no. 574/72 laying down detailed rules for the application of Regulation (EEC) no. 1408/72, for the following categories:
• workers and their family members who, after having acquired the right to health insurance in an EU Member State, are authorized by the National Health Insurance of that State to return to the territory of the EU Member State in which they reside or to transfers his residence to the territory of another Member State;
• all insured persons and their family members who are authorized by the Health Insurance House to travel to the territory of another EU Member State to receive there treatment appropriate to their state of health.
When performing medical treatments, patients must meet two cumulative conditions:
1. medical treatment is found among the medical services provided in the package of basic services that the insured benefit from the social health insurance system in Romania;
2. the treatment cannot be granted to the patient in the time interval normally necessary to obtain the respective treatment in Romania, taking into account the state of health at the moment and the probable evolution of the disease.
It is also very important to know that:
• the hospital / clinic where the patient wishes to undergo medical treatment must be located in a member country of the European Union or Switzerland;
• the hospital / clinic must have a contract with the National Health Insurance House of the respective country.
In order to obtain the S2 form, patients must complete the following steps:
1. translation of medical documents into either English or the language of the country where the hospital or clinic is located;
2. sending the translated file to the specialist doctor from abroad and obtaining the agreement from the hospital / clinic;
3. The patient’s letter of acceptance for treatment issued by the hospital / clinic abroad must contain the following information: patient’s name, diagnosis, medical services to be provided, date the hospital / clinic receives the patient, estimated time for completion treatment, respectively the agreement for S2 Form in order to settle the costs of the treatment.
Attention: some hospitals / clinics may require patients, at the time of hospitalization, to transfer an amount to the hospital / clinic account, as a guarantee for the situation in which certain investigations are required that cannot be supported by S2 Form. These details are not filled in in the acceptance letter, but are sent separately by the hospital / clinic.
4. translation of the letter of acceptance into Romanian (in this sense, the translation must be authorized and its notarial authentication is not required).
5. submitting a written application to the Health Insurance House in the county of residence requesting S2 Form (http://www.cnas.ro/casmb/page/formulare-europene.html). The file submitted must contain, in addition to the written request, the following documents:
• copy of the identity document or birth certificate in the case of minors – copies and originals for compliance;
• legalized power of attorney, if the request is made by another person on behalf of the patient;
• proof of the quality of the insured, as the case may be;
• medical file that will contain medical documents: copies of medical documents and medical report – http://www.cnas.ro/casmb/media/files/Formular_E112_raport_medical.pdf;
• the letter of acceptance from the hospital / clinic in the EU or EEA Member State in which it is intended to perform the medical treatment or medical service that the patient needs, respectively the availability to provide the treatment, respectively the medical service, based on S2 Form, as well as specifying the period in which the treatment will be performed;
• authorized translation of the confirmation.
6. The application accompanied by the above-mentioned documents shall be submitted to the registry of the National Health Insurance House.
The application for S2 Form is issued within a maximum of 5 working days.
S2 form is issued before the patient leaves for treatment abroad. However, the Form may also be issued after the beneficiary has left abroad, in the following situations:
• in case of force majeure, the document cannot be issued in advance;
• the value of the medical services was not paid by the patient before the form was issued.
In this case, the Insurance House draws up a report arguing and justifying the situation of force majeure, a document that will be the basis for the issuance of S2 form (formely E112). Benefits based on S2 form (E112) are settled only between institutions, based on invoices.
